قياس سمك الطبقة الداخلية والوسطى في الشريان السباتي بواسطة الموجات الصوتية كاستنباط لوجود مرض القصور بالشرايين التاجية

ملخص الدراسة:

حيث ان أمراض ضيق الشرايين التاجية من اخطر أسباب الوفاة في العصر الحديث والاستنباط المبكر قبل ظهور الأعراض يؤدى إلى العلاج الوقائي المبكر مما يؤخر ظهور الأعراض.

فى هذا البحث الذى أقيم بمستشفيات جامعة عين شمس لدراسة نوع جديد من المستنبطات؛ نقوم بدراسة السمك الجداري للطبقتين الداخلية والوسطي للشريان السباتى باتخدام الموجات الصوتية الوعائية عالية القدرة على التفريق، كآداة لاستنباط امراض الشرايين التاجية، ومحاولة للربط بين سمك الجدار وعناصر الخطر التى تؤدى لحدوث امر اض ضيق الشرايين التاجية.

فقد تم دراسة مئة مريض منهم اربع وعشرين لديهم شرايين تاجية سليمة والست وسبعين الباقون لديهم ضيق باشرايين التاجية متضح عن طريق القسطرة القلبية التشخيصية، والتصوير الصبغى للشرايين التاجية.

فقد تم اخذ متوسط قياسات متعددة للسمك الجدارى للطبقتين الداخلية والوسطي بالجزء الاخير من الشريان السباتى من النصف الجدارى البعيد عن اداة الموجات الصوتية ووجد انها على ارتباط شديد مع عناصر الخطر التى تؤدى لحدوث امر اض ضيق الشرايين التاجية وخاصة السن، ارتفاع نسبة الدهون في الدم، السمنة، ارتفاع ضغط الدم، وقد وجد ان معدل سمك الجدار يكون اكثر بالمدخنين عن غير المدخنين.

و بصدد اثبات استخدام سمك الجدار كأداة لاستنباط معدل حدوث وشدة الاصابة بالشرايين التاجية، وجد ان هناك ارتباط وثيق بين شمك الجدار ونسبة الاصابة بأمراض الشرايين التاجية بنسبة حساسية تصل إلى 87.5% ودقة بلغت نسبتها 85% عند استخدام سمك جداري 0.7مم في الجزء الأبعد من شريان السباتى الرئيسي بواسطة موجات صوتية شريانية عالية التفريق.

هذا بلاضافة إلى قدرة السمك الجداري للطبقة الداخلية والوسطى للشريان الراوي الرئيسي المستنبطة بالموجات الصوتية شريانية عالية التفريق على استنباط شدة الاصابة بأمراض الشرايين التاجية ومدى انتشار مرض تصلب الشرايين داخل شرايين القلب."

Ain Shams University"               "TEE technology has rapidly evolved with the advancements provided by flexibe endoscopic probes and phased array transducers ultrasound technology.

      The additional information available from pulsed wave, continuous wave, and colour Doppler data has led to the rapid acceptance of TEE by the medical, surgical and anesthetic communities. TEE is a diagnostic examination with an ultrasound device passed to the oesophagus to create a clear image of the cardiac muscle and other parts of the heart.

      TEE probes are variable, but share several common features, there are adult and pediatric sizes, it can be introduced in Awake patient with local anesthetic and sedation or in anesthetized patients.

      The tips of all TEE transducers can be flexed to some degree both antroposteriorly and laterally to accommodate the different angles that may be required to visualize various parts of the heart and great vessels. A refrence is usually displayed on the screen indicating longitudinal (L) or transverse (T) plane.

      In transverse plane (0) the probe is moved up and down in the oesophagus in order to obtain the standard imaging planes. While imaging in the longitudinal plane, the operator rotates the probe clockwise and counterclockwise along its long axis."

Ahmed Mohammed Khamis"              "In recent years there has been a growing interest in the practice of regional anesthesia and ,in particular, peripheral nerve blocks for surgical anesthesia and postoperative analgesia. Peripheral nerve blocks have been found to be superior to general anesthesia, as they provide effective analgesia with few side effects, and can fasten patient recovery.

Unfortunately the practice of regional anesthesia does not enjoy widespread endorsement because of inconsistent success, varying from one anesthesiologist to another. Current methods of nerve localization (e.g, parathesia and nerve stimulation) are essentially blind procedures, since they both relay on an indirect evidence in needle- to- nerve contact.

In this essay we threw light on application of ultrasound in blockage of the brachial plexus by four techniques which are interscalene, supraclavicular, infraclavicular and axillary brachial plexus block.

 

Seeking nerves by trial and error and random needle movement can cause complications. Although uncommon, complications such as intravascular local anesthetic injection resulting in systemic toxicity, inadvertent spinal cord injury following interscalene block, pneumothorax following supraclavicular block, and nerve injury have been all reported.
