التغيرات الدماغية الناشئة عن حالات إصابات الرأس المغلقة

الملخص العربي:
يعتبر أذى الرأس مشكلة صحيه كبرى و هو ينشأ عن أي إصابة للرأس. إصابات الرأس إما أن تكون إصابات مفتوحه أو إصابات مغلقة. الأذى الإصابى للمخ يشمل العديد من العوامل التي تنشأ من استعمال قوه ميكانيكيه على الرأس.
و قد هدفت هذه الدراسة إلى تحديد التغيرات الظاهرية و المجهرية فى الدماغ فى حالات إصابات الرأس المغلقة و العلاقه بين هذه التغيرات و بين كل من سن وجنس الحالات التى درست و ظروف الإصابه و فترة الحياه بعد الإصابه و التداخلات الطبيه و الجراحيه و وجود إصابات أخرى أو عقاقير الإدمان و الأمراض المصاحبه و ذلك فى منطقة القاهره الكبرى. و قد تم جمع و تحليل المعلومات الخاصه بخمسين حاله من حالات إصابات الرأس المغلقه فى هذه المنطقه.
تم تقسيم الحالات حسب العمر إلى عقود و قد وجدت معظم الحالات فى العقد الثالث (14 حاله) و الرابع (10 حالات) و الثانى (9 حالات). كما وجد أن معظم الحالات كان من الذكور (64%).
معظم الحالات (80%) تعرضت لإصابه مباشره على الرأس بينما فى 20% من الحالات كانت الإصابه غير مباشره. كما كانت معظم الحالات (92%) قد أصيبت للمرة الأولى.
أسباب إصابات الرأس فى هذه الدراسه كانت التعرض لإعتداء عنيف (76%)، أو السقوط من وضع الوقوف أو من إرتفاع أقصاه متر واحد (14%) أو حوادث السيارات (10%).
معظم الحالات (72%) أصيبت نتيجة إعتداء جنائى بينما وجدت حاله واحده فقط أحدث المتوفى بها إصابته بنفسه.
حالات الدراسه كانت توجد بها إصابات خارجيه بالرأس على هيئة تكدمات (51.92%) و جروح مفتوحه (19.23%) بينما 28.85% من الحالات كانت بدون إصابات خارجيه بالرأس.
معظم الحالات (70%) توفيت فى خلال 24 ساعه بعد الإصابه و كانت أطول فترة بقاء بعد الإصابه هى 45 يوماً. كما وجد أن 76% من الحالات لم تتلقى أى علاج.
الإصابات الخارجيه الأخرى وجدت معظمها فى الأطراف (31.46%)، و فى الوجه و العنق (25.48%). معظم الحالات (88%) لم تكن بها إصابات داخليه أخرى. كذلك كانت توجد حاله واحده فقط متعاطيه للكحول. الأمراض المصاحبه وجدت فى 21 حاله فقط.
التغيرات الباثولوجيه الظاهريه التى وجدت فى أدمغة الحالات المدروسه كانت على هيئة إحتقان (38 حاله)، إرتشاح (24 حاله)، نزيف داخل تجويف الجمجمه (19 حاله)، تكدمات (7 حالات)، تهتكات (حالتان)، نخر (حاله واحده) و تقيح (حاله واحده). النوع الأكثر شيوعاً من النزيف الدماغى كان النزيف تحت العنكبوتيه (10 حالات).
وجد أن الإحتقان و الإرتشاح و النزيف هى المظاهر الوحيده المشاهده فى المجموعات العمريه المتطرفه. كما لم يعثر على تهتكات أو نخر أو تقيح فى الإصابه غير المباشره.
كما وجد أن الحاله الوحيده المصابه بالتقيح كان بها جرح بالفروه. و كذا النخر و التقيح وجدا فقط فى الحالات التى عاشت لفتره تزيد عن أسبوع بعد الإصابه.
تم إستخدام صبغات مختلفه لإظهار التغيرات المجهريه فى النسيج الدماغى. تم صبغ القطاعات البارافينيه المأخوذه من الدماغ بواسطة صبغة الهيماتوكسلين و الإيوسين و كذا بالصبغات الخاصه و هم: صبغة مايسون ترايكروم لإظهار الكولاجين، صبغة الفضه لإظهار الألياف العصبيه، صبغة أزرق و قرمزى ماريتوس لإظهار النسيج الليفى و صبغة أزرق بروسيا لإظهار حديد الدم.
التغيرات المجهريه فى أدمغة حالات الدراسه وجدت على هيئة: إحتقان (40 حاله)، بؤر إرتشاحيه (14 حاله)، إ رتشاح عام (24 حاله)، نقط نزفيه (حالتان)، بؤر نزفيه (22 حاله)، إصابه بؤريه بالألياف العصبيه (9 حالات)، إصابه منتشره بالألياف العصبيه (11 حاله)، تغيرات ناشئه عن نقص الأكسجين (11 حاله)، تهتكات (حاله واحده)، نخر (6 حالات) و إحتشاء (حالتان).
شوهد الإحتقان على هيئة إتساع الأوعيه الدمويه البينيه و إمتلائها بكرات الدم الحمراء.
شوهد الإرتشاح على هيئة تفكك النسيج البينى و إتساع الفراغات حول الأوعيه الدمويه. و كان الإرتشاح إما منتشراً أو متمركزاً حول التغيرات الباثولوجيه بنسيج المخ.
النزيف البؤرى وجد أغلبه فى الحالات التى ماتت بعد الإصابه مباشرة. و وجد أغلبه فى المخ يتبعه جسر المخ. كما عثر أيضاً على النزيف العام بالفحص المجهرى.
إصابة الألياف العصبيه شوهدت على هيئة إنتفاخ الألياف (ألياف عصبيه سميكه و متعرجه)، تكون منتفخ و كرات إنكماشيه. الإصابه البؤريه للألياف العصبيه وجد أغلبها فى جذع المخ. الإصابه المنتشره للألياف العصبيه صنفت حسب موقعها إلى مرحله أولى، مرحله ثانيه و مرحله ثالثه.
التغيرات الناشئه عن نقص الأكسجين وجدت على هيئة زيادة القابليه للصبغه الحمضيه و خلايا عصبيه حمراء. وقد وجدت فى المخ غالباً فى مناطق الحواجز و فى حصان البحر. أما فى المخيخ فقد وجدت فى خلايا بركنجى.
النخر وجد إما متسيل أو نزفى. و كذا الإحتشاء وجد إما نزفى أو عير نزفى.
Summary and Conclusion
Head injury is a major health problem that results from any trauma to the head. Head injury may be open or closed head injury. Traumatic brain injury refers to a broad range of neurological factors that result from the application of a mechanical force to the head .
The current study was conducted to determine the gross morphological and microscopical changes of the brain in cases of closed head injury and the relation between these changes and each of age and gender of the studied cases, the circumstances of the trauma, post-traumatic survival period, interventions, associated trauma or drugs of abuse and associated diseases, in Greater Cairo Area. Data of 50 cases of closed head injury in this area were collected and analyzed.
The cases were grouped according to their ages into decades. We found the majority of cases were in the 3rd decade (14 cases), 4th decade (10 cases) and 2nd decade (9 cases). It was found also that the majority of cases (64%) were males.
The majority of cases (80%) sustained direct trauma while 20% of cases were injured by indirect trauma. Also, the majority of cases (92%) were injured for the first time.
Causes of head trauma in this study were violent assaults (76%), falling on the head from the standing position or a height of a maximum of one meter (14%) and car accidents (10%).
The majority of the cases (72%) were injured in homicidal attacks while there was only one case of self-inflicted head injury.
The cases had external head injuries in the form of bruises (51.92%) and open wounds (19.23%) with 28.85% of the cases with no external head injuries.
The majority of the cases (70%) died within 24 hours after the injury. The longest survival time was 45 days. It was also found that 76% of the cases received no treatment.
Associated external traumas were found mostly in the limbs (31.46%) and in the face and neck (25.84%). The majority of cases (88%) didn't have other internal body injuries. There was also one case with alcohol intake. Associated diseases were found only in 21 cases.
Gross pathological changes in the brains of the studied cases were found in the form of; congestion (38 cases), edema (24 cases), intracranial hemorrhage (19 cases), bruises (7 cases), lacerations (2 cases), necrosis (one case) and infection (one case). The commonest type of intracranial hemorrhage was SAH (10 cases).
It was found that congestion, edema and hemorrhage were the only findings in the extremes of age. In cases with indirect trauma, there were no lacerations, necrosis or infection in these cases.
It was found also that the only case with infection had a scalp wound. Also, necrosis and infection were only found in cases surviving for more than one week.
Different stains were used for demonstration of histopathological changes in the brain tissue. Paraffin sections from the brain were prepared and stained with Hematoxylin and Eosin and also with special stains which were: Masson’s trichrome stain for demonstration of collagen, silver stain for demonstration of axons, MSB (Martius scarlet blue) stain for demonstration of fibrin and Perl's Prussian blue for demonstration of hemosiderin.
Microscopic changes in the brains of the studied cases were found as; congestion (40 cases), focal edema (14 cases), generalized edema (24 cases), petechial hemorrhages (2 cases), focal hemorrhage (22 cases), focal AI (9 cases), DAI (11 cases), hypoxic-ischemic changes (11 cases), lacerations (one case), necrosis (6 cases) and infarction (2 cases).
Congestion was seen as dilated interstitial blood vessels filled with RBCs.
Edema was seen as meshing of the interstitium and widening of spaces around the blood vessels. It was either generalized or around different pathological changes in the brain tissue.
Focal hemorrhage was mostly found in cases with immediate death. It was mostly found in the cerebrum followed by pons. Generalized hemorrhage was also seen microscopically
Axonal injury was seen as axonal swelling (thick tortuous axons), bulb formations and retraction balls. Focal axonal injury was found mostly in the brainstem. Diffuse axonal injury was graded according to its site into Grade I, grade II and grade III.
Hypoxic-ischemic changes were found in the form of esinophilia and red neurons. It was found in the cerebrum mostly in the watershed areas and hippocampus. In the cerebellum it was found in purkenji cells.
Necrosis was found as either liquifactive or hemorrhagic. Also, infarction was found as hemorrhagic and non-hemorrhagic.
